Population Health in the Practice: The New Medical Model: A Primary Care Physician’s Perspective


Location: Auditorium at Cooper Medical School of Rowan University 

Primary care is at the nucleus of contemporary patient- centered healthcare. Approaches like the Patient Centered Medical Home (PCMH) have required primary care physicians and care teams to rework traditional delivery models. Dr. Allan Crimm, Physician with 9th Street Internal Medicine (an NCQA recognized level three PCMH), will kick-off our fall series with a riveting discussion of his experience implementing the PCMH model and his reflections on its challenges to him and his care team.
